NOTE REGARDING PAYPAL CHECKOUT: When you get to the end of the PayPal payment screen, Paypal automatically prompts you to create an account if you do not have one. YOU DO NOT NEED TO CREATE AN ACCOUNT. Simply Choose the "Continue as Guest" option as shown in this picture.

You now have the option of registering and making your payments online by using the below form OR you can print out a form to fill out and return it with your check. The Online form will calculate your registration fees and then you will select the "Checkout with Paypal" button that will appear at the bottom of the form once you're done filling it out. You will then be taken to Paypal where you should see options like this picture where you can either pay through an existing PayPal account or with your credit card.
